How did Ghostmaker chop Batman's arm off?

Recently, Batman has a robot hand. The reason he lost his hand is because of Ghostmaker, a vigilante who works in Gotham, however, this Ghostmaker wasn't the normal one, he was from another universe which Batman got transported to because of Failsafe (a robot designed to kill Batman).

Back in our current timeline, Failsafe attacked Bruce and the Batfamily because Batman was accused of killing Oswald Cobblepot, the Penguin. Failsafe tracked Bruce to Wayne Manor and attacked him, with most of the Batfamily incapacitated, only Tim Drake could help.

By the time Failsafe got there, Bruce Wayne wasn't in control, Zurr En Arr was. Zurr En Arr is a version of Batman who does not have a Bruce Wayne, he's an alternate personality and is more brutal than Bruce. Zurr and Tim fought Failsafe, but the backup personality of Zurr thought Robin as a soldier, not family. Bruce then went back in control of his body and fought Failsafe some more, before being defeated. As the robot tried to kill Batman, The Man of Steel showed up.

After Superman appeared, Failsafe decided to outsmart him by flying away, only to be chased by Superman, however, Failsafe planned for this and decided to stab him with Kryptonite blades once they were back at Wayne Manor. Then, Hawkgirl, Black Canary, Green Arrow and Martian Manhunter attacked Failsafe. Whilst they were fighting, Tim Drake evacuated Batman and Superman into a jet. Back to the fight, Failsafe moved to an area of Gotham filled with contingencies against the Justice League, owned by Bruce Wayne, he made Hawkgirl go flying into a magnetized building, he shot a gas mask on Black Canary that makes her unable to scream and finally, Martian Manhunter was attacked with fire. Nightwing decided to help Green Arrow out, but they didn't stand a chance.

Batman decided to jump out the jet they was in whilst above the ocean, he passed out and woke up two weeks later in Atlantis, Batman suited up, but Failsafe attacked shortly after. Aquaman evacuated Batman and was stabbed by Failsafe, trying to defend Bruce. Batman escaped, going to the Watchtower, Failsafe attacked, but Bruce self destructed the section they was in, whilst teleporting Failsafe to Earh. Batman was stranded in space, but he made his way to Earth and landed at the Fortress Of Solitude. Robin and Superman were there, and created a microchip that changed Failsafe's personality. After Superman was incapacitated, Batman and Robin activated the chip, Failsafe ran to the Fortress and took a gun that disintegrated Batman.

Batman then woke up in Gotham, his suit destroyed and no gear. He realized that there was no Batman or Joker in this universe, and Red Mask was in control. Red Mask is a dictator who can jump split seconds in time, he fought Batman who created his own suit from scratch and ran away, then, Ghostmaker attacked Bruce, Ghostmaker was on the venom drug and chopped Bruce's hand off, Batman then used bandages and wrapped his hand up, then defeated Ghostmaker and broke the world free of Red Mask, chasing him through the Batman Beyond universe, Batman The Animated Series universe, the Arkham universe, Keaton's universe, a vampire universe, The Dark Knight Returns universe and even Adam West's universe.
